Medical term for PlasmaTherapy is = Convalescent Plasmapheresis

If you are looking forward to obtain Plasma from a Blood Bank, you would need following documentation and processes to be fulfilled and adhered:

Blood Requisition Form - 

This form may either be issued by Hospital to the BloodBank or sometimes BloodBank provides this form for the hospital to fill-in. You should first ask BloodBank to give you this form, as they have their own format and might not necessarily take the Hospital Requisition Form. 

Patient's Blood Sample -

Blood sample in 2 different vials with Red and Purple Cap is also required. Hospital requesting for plasma would provide the patient's blood sample. Using this sample only the BloodBank would prepare and issue the Cross-Match form to the treating Hospital.

Patient's Blood Group Report -

This is also needed issued from a pathology lab where patient is admitted or some old report also works. 

Cross-Match Form - 

This form is issued by Blood Bank after matching the Donor's and Recipient's Blood Samples. This is a mandatory form to be filled by Blood Bank otherwise treating Hospital might reject the Plasma.

 

One more thing, while you are taking away the Plasma from Blood Bank, ensure that they keep a sample of same plasma in their library in case of any conflicts or to reissue the cross-match form.

COST - 

1. KGMC, Lucknow - Charging 14000/- Rs for plasma packet to issue to the Pvt. Hospital.
2. GSVM, Kanpur - You need to buy or pay the Plasma Container Kit costing 7500/- Rs from authorised dealers as recommended by Blood Bank. For internal usage in Hallet, they might waive-off this charge but for Pvt. you need to pay this amount.
